I do not complete emotional support animal (ESA) letters.

Peach Moon Mental Health, LLC reserves the right to terminate the provider-patient relationship under the following conditions:

• When a client has frequent missed appointments or late cancellations (>2 consecutive missed appointments or >4 total in a calendar year)

• When a client or family member engages in hostile, aggressive, or abusive behavior towards the provider

• Due to non-payment of services or deliberate negligence in updating payment information

• When a client is not adhering to treatment recommendations or is deemed to no longer benefit from services offered by the provider

• When the provider or Peach Moon Mental Health, LLC is not able to meet the needs of the client
